Si un nombre croissant de travaux ont explor\u00e9 l\u2019association entre la qualit\u00e9 du r\u00e9gime alimentaire et le risque de diab\u00e8te de type 2, peu d\u2019\u00e9tudes ont \u00e9t\u00e9 men\u00e9es chez des populations rurales. Pour y pallier, une \u00e9tude r\u00e9cente a analys\u00e9 la relation dose-r\u00e9ponse entre la consommation de fruits et l\u00e9gumes et le risque de diab\u00e8te de type 2 en Chine rurale. Ce travail montre qu\u2019une consommation \u00e9lev\u00e9e de fruits seuls ou de fruits et l\u00e9gumes combin\u00e9s est li\u00e9e \u00e0 un risque r\u00e9duit de diab\u00e8te de type 2. Il invite \u00e0 encourager la consommation des fruits et l\u00e9gumes dans le cadre d\u2019habitudes de vie globalement saines.<\/strong><\/p>\n\n\n\n<p>Au cours des <strong>30 derni\u00e8res ann\u00e9es<\/strong>, le <strong>d\u00e9veloppement \u00e9conomique<\/strong> a <strong>grandement boulevers\u00e9<\/strong> le <strong>mode de vie<\/strong> &#8211; en particulier le <strong>r\u00e9gime alimentaire et le niveau d\u2019activit\u00e9 physique\/ s\u00e9dentarit\u00e9<\/strong> &#8211; de la population chinoise (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29111090\/\" target=\"_blank\" rel=\"noreferrer noopener\">Schwingshackl, 2017<\/a>). La<strong> consommation<\/strong> de<strong> viande<\/strong>, de<strong> produits laitiers<\/strong>, de <strong>c\u00e9r\u00e9ales raffin\u00e9es<\/strong> et d<strong>&lsquo;aliments transform\u00e9s<\/strong> a notamment <strong>progressivement augment\u00e9<\/strong>, au d\u00e9triment de la <strong>consommation de fruits et l\u00e9gumes, soja <\/strong>et <strong>noix<\/strong> (<a href=\"https:\/\/journals.plos.org\/plosone\/article?id=10.1371\/journal.pone.0195775\" target=\"_blank\" rel=\"noreferrer noopener\">Chang, 2018<\/a>\u202f; <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7432933\/\" target=\"_blank\" rel=\"noreferrer noopener\">Han, 2020<\/a>).<\/p>\n\n\n\n<p>En parall\u00e8le, de l\u2019\u00e9volution de ces modes de vie, la <strong>pr\u00e9valence des maladies chroniques, notamment du diab\u00e8te de type 2 a tr\u00e8s largement augment\u00e9<\/strong> passant de moins de 1% de la population en 1980 \u00e0 12,8% en 2017 (Li, 2020).<\/p>\n\n\n\n<p>Parmi les facteurs de risques identifi\u00e9s de diab\u00e8te de type 2 figurent l\u2019exc\u00e8s de poids et le manque d\u2019activit\u00e9 physique. Une alimentation saine riche en fruits et l\u00e9gumes fait ainsi partie des leviers de pr\u00e9vention recommand\u00e9 vis-\u00e0-vis de cette pathologie. Cependant, <strong>bien qu\u2019elles sugg\u00e9rent un effet protecteur<\/strong>, les \u00e9tudes ayant explor\u00e9 le <strong>lien<\/strong> entre<strong> consommation<\/strong> de <strong>fruits<\/strong> et <strong>l\u00e9gumes <\/strong>et risque de <strong>diab\u00e8te de type 2<\/strong>, ne permettent pas d\u2019<strong>\u00e9tablir de consensus<\/strong> sur cette <strong>association <\/strong>(<a href=\"https:\/\/www.bmj.com\/content\/370\/bmj.m2194\" target=\"_blank\" rel=\"noreferrer noopener\">Li, 2014\u202f<\/a>; <a href=\"https:\/\/www.bmj.com\/content\/370\/bmj.m2194\" target=\"_blank\" rel=\"noreferrer noopener\">Zheng, 2020<\/a>\u202f; <a href=\"https:\/\/nutrition.bmj.com\/content\/4\/2\/519\" target=\"_blank\" rel=\"noreferrer noopener\">Halvorsen, 2021<\/a>).<\/p>\n\n\n\n<p>Enfin, les <strong>donn\u00e9es<\/strong> portant sur les<strong> communaut\u00e9s rurales<\/strong> en Chines restent <strong>tr\u00e8s limit\u00e9es<\/strong>. Afin de <strong>fournir plus de preuves<\/strong>, une \u00e9tude r\u00e9cente (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/36739200\/\" target=\"_blank\" rel=\"noreferrer noopener\">Niu, 2022<\/a>) a explor\u00e9 la <strong>relation dose-r\u00e9ponse<\/strong> entre la <strong>consommation de fruits et l\u00e9gumes<\/strong> et le <strong>risque de diab\u00e8te de type 2<\/strong> en Chine rurale (voir m\u00e9thodologie).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Des tableaux biologiques et cliniques diff\u00e9rents entre sujets sains et personnes atteintes de diab\u00e8te de type 2<\/h2>\n\n\n\n<p>Dans un premier temps, des<strong> analyses biologiques<\/strong> et<strong> cliniques<\/strong> ont \u00e9t\u00e9 effectu\u00e9es puis <strong>compar\u00e9es<\/strong> entre les personnes souffrant de <strong>diab\u00e8te de type 2 <\/strong>et les<strong> sujets sains<\/strong>.<\/p>\n\n\n\n<p>D\u2019apr\u00e8s ce travail, les personnes atteintes de <strong>diab\u00e8te de type 2<\/strong> \u00e9taient comparativement :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Plus \u00e2g\u00e9es ;<\/li>\n\n\n\n<li>Moins actives physiquement ;<\/li>\n\n\n\n<li>Des niveaux d\u2019\u00e9ducation et de revenus plus faibles.<\/li>\n<\/ul>\n\n\n\n<p>Elles pr\u00e9sentaient \u00e9galement :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Des <strong>param\u00e8tres anthropom\u00e9triques et m\u00e9taboliques plus \u00e9lev\u00e9s<\/strong> : tour de taille et de hanches, IMC, glyc\u00e9mie, insulin\u00e9mie, LDH-cholest\u00e9rol et cholest\u00e9rol total, triglyc\u00e9rid\u00e9mie ;<\/li>\n\n\n\n<li>De l\u2019<strong>hypertension<\/strong> et des <strong>dyslipid\u00e9mies plus fr\u00e9quente<\/strong>s ;<\/li>\n\n\n\n<li>Des <strong>apports en glucides<\/strong> et <strong>apports \u00e9nerg\u00e9tiques totaux moindres<\/strong> ;<\/li>\n<\/ul>\n\n\n\n<p>Enfin, elles \u00e9taient<strong> moins susceptibles <\/strong>de <strong>fumer<\/strong> et de <strong>consommer<\/strong> de l\u2019<strong>alcool<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Un risque r\u00e9duit de diab\u00e8te de type 2 observ\u00e9 chez les forts consommateurs de fruit et de fruits et l\u00e9gumes<\/h2>\n\n\n\n<p>Une analyse statistique du risque de diab\u00e8te a ensuite \u00e9t\u00e9 effectu\u00e9e selon le niveau de consommation de fruits et l\u00e9gumes des participants. D\u2019apr\u00e8s ce travail, une <strong>corr\u00e9lation<\/strong> entre une <strong>consommation accrue<\/strong> de <strong>fruits<\/strong> et la <strong>diminution<\/strong> du <strong>risque<\/strong> de <strong>diab\u00e8te de type 2 a \u00e9t\u00e9 observ\u00e9e<\/strong>, celle-ci pouvant aller jusqu\u2019\u00e0 <strong>59,1 %<\/strong>.<\/p>\n\n\n\n<p>De plus, l&rsquo;analyse <strong>dose-r\u00e9ponse<\/strong> montre que les participants issus de la<strong> Chine rurale<\/strong> qui <strong>consomment plus de 260 g de fruits par jour<\/strong> ont un <strong>risque plus faible<\/strong> de d\u00e9velopper un <strong>diab\u00e8te de type 2<\/strong>, tandis que ceux <strong>qui en consomment moins pr\u00e9sentent un risque plus \u00e9lev\u00e9<\/strong>. Ces r\u00e9sultats sont<strong> conformes<\/strong> aux <strong>conclusions<\/strong> de nombreuses \u00e9tudes sur l\u2019association entre une<strong> consommation importante<\/strong> de <strong>fruits<\/strong> et le risque de<strong> diab\u00e8te de type 2<\/strong> (<a href=\"https:\/\/nutrition.bmj.com\/content\/4\/2\/519\" target=\"_blank\" rel=\"noreferrer noopener\">Halvorsen, 2021<\/a> ; <a href=\"https:\/\/www.bmj.com\/content\/370\/bmj.m2194\" target=\"_blank\" rel=\"noreferrer noopener\">Li, 2014 <\/a>; <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/18390796\/\" target=\"_blank\" rel=\"noreferrer noopener\">Bazzano, 2008<\/a>).<\/p>\n\n\n\n<p>Une autre <strong>corr\u00e9lation<\/strong> a \u00e9galement \u00e9t\u00e9 observ\u00e9e entre la <strong>consommation conjointe de fruits et l\u00e9gumes <\/strong>et le <strong>risque de diab\u00e8te de type 2<\/strong>. En effet, dans cette \u00e9tude, une <strong>consommation plus importante de fruits <\/strong>et<strong> l\u00e9gumes<\/strong> a \u00e9t\u00e9 associ\u00e9e \u00e0 une r\u00e9duction de 34,2% du <strong>risque de diab\u00e8te de type 2<\/strong>. Apr\u00e8s analyse de la relation dose-r\u00e9ponse, le <strong>risque de diab\u00e8te de type 2 <\/strong>a diminu\u00e9 de <strong>mani\u00e8re significative<\/strong> lorsque la <strong>consommation de fruits et l\u00e9gumes<\/strong> \u00e9tait comprise entre<strong> 600 et 1 000 g\/jour <\/strong>chez les personnes vivant dans la Chine rurale. Toutefois, <strong>aucune association significative<\/strong> n&rsquo;a \u00e9t\u00e9 observ\u00e9e pour une <strong>consommation sup\u00e9rieure \u00e0 1000 g\/jour<\/strong>. De m\u00eame, ces r\u00e9sultats sont<strong> conformes<\/strong> \u00e0 ceux d\u2019autres mis en \u00e9vidence dans d\u2019autres \u00e9tudes (<a href=\"https:\/\/nutrition.bmj.com\/content\/4\/2\/519\" target=\"_blank\" rel=\"noreferrer noopener\">Halvorsen, 2021<\/a> ; <a href=\"https:\/\/www.bmj.com\/content\/370\/bmj.m2194\" target=\"_blank\" rel=\"noreferrer noopener\">Zheng, 2020<\/a> ; <a href=\"https:\/\/diabetesjournals.org\/care\/article\/35\/6\/1293\/38898\/A-Prospective-Study-of-the-Association-Between\" target=\"_blank\" rel=\"noreferrer noopener\">Cooper, 2012<\/a>).<\/p>\n\n\n\n<p>Contrairement \u00e0 d&rsquo;autres recherches men\u00e9es dans les pays occidentaux (<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0022316622095906?via%3Dihub\" target=\"_blank\" rel=\"noreferrer noopener\">Villegas, 2008<\/a> ; <a href=\"https:\/\/diabetesjournals.org\/care\/article\/35\/6\/1293\/38898\/A-Prospective-Study-of-the-Association-Between\" target=\"_blank\" rel=\"noreferrer noopener\">Cooper, 2012<\/a>), ce travail n&rsquo;a \u00e9tabli <strong>aucune association significative<\/strong> entre la <strong>consommation de l\u00e9gumes<\/strong> et le <strong>diab\u00e8te de type 2<\/strong>. Ce constat peut s&rsquo;expliquer par la <strong>diversit\u00e9 culturelle<\/strong> et les <strong>disparit\u00e9s<\/strong> dans les <strong>habitudes alimentaires<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Des effets associ\u00e9s \u00e0 la pr\u00e9sence de nombreux compos\u00e9s bioactifs dans les fruits et l\u00e9gumes<\/h2>\n\n\n\n<p>Selon les auteurs, l&rsquo;<strong>association inverse<\/strong> observ\u00e9e entre la<strong> consommation de fruits<\/strong> et le <strong>risque de diab\u00e8te de type 2<\/strong> peut s&rsquo;expliquer par le <strong>faible indice glyc\u00e9mique<\/strong> des<strong> fruits<\/strong> les plus <strong>couramment consomm\u00e9s <\/strong>dans la Chine rurale &#8211; pommes, raisins, bananes &#8211; (<a href=\"https:\/\/link.springer.com\/article\/10.1007\/s00125-010-1927-1\" target=\"_blank\" rel=\"noreferrer noopener\">Jenkins, 2011<\/a>).<\/p>\n\n\n\n<p>L&rsquo;effet de la <strong>consommation conjointe<\/strong> de <strong>fruits <\/strong>et <strong>l\u00e9gumes<\/strong> peut quant \u00e0 lui s\u2019expliquer par le <strong>large \u00e9ventai<\/strong>l de <strong>nutriments <\/strong>et de <strong>compos\u00e9s<\/strong> pr\u00e9sents dans les fruits et l\u00e9gumes et <strong>contribuant \u00e0 la pr\u00e9vention <\/strong>du <strong>diab\u00e8te de type 2<\/strong> :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Les <strong>antioxydants<\/strong> tels que les <strong>vitamines A, C<\/strong> et les<strong> carot\u00e9no\u00efdes<\/strong> jouent un <strong>r\u00f4le majeur<\/strong> dans l&rsquo;<strong>\u00e9limination des radicaux libres<\/strong> produits au cours du m\u00e9tabolisme. Ces compos\u00e9s contribueraient ainsi \u00e0 <strong>limiter<\/strong> le <strong>stress oxydatif<\/strong> sur les cellules \u03b2 des \u00eelots de Langerhans, et \u00e0 <strong>pr\u00e9server la stabilit\u00e9 <\/strong>de la <strong>glyc\u00e9mie<\/strong> (<a href=\"https:\/\/link.springer.com\/article\/10.1186\/2251-6581-12-43\" target=\"_blank\" rel=\"noreferrer noopener\">Bahadoran, 2013<\/a>).<\/li>\n\n\n\n<li>Les <strong>fibres<\/strong> via la<strong> modification<\/strong> du <strong>microbiome intestinal<\/strong>, participeraient \u00e0 l&rsquo;<strong>am\u00e9lioration<\/strong> de la <strong>sensibilit\u00e9<\/strong> \u00e0 l&rsquo;<strong>insuline<\/strong> et<strong> limitent<\/strong> l\u2019apparition d\u2019<strong>insulinor\u00e9sistance<\/strong>,<strong> r\u00e9duisant<\/strong> ainsi le <strong>risque de diab\u00e8te de type 2<\/strong> (<a href=\"https:\/\/www.thelancet.com\/journals\/ebiom\/article\/PIIS2352-3964(19)30800-X\/fulltext\" target=\"_blank\" rel=\"noreferrer noopener\">Gurung, 2020<\/a>).<\/li>\n\n\n\n<li>Les <strong>mol\u00e9cules bioactives anti-inflammatoires<\/strong> et les<strong> min\u00e9raux<\/strong> tels que le magn\u00e9sium, <strong>limiteraient<\/strong> la <strong>r\u00e9sistance \u00e0 l&rsquo;insuline<\/strong>, <strong>r\u00e9duisant<\/strong> ainsi le <strong>risque de d\u00e9velopper un diab\u00e8te de type 2<\/strong> (<a href=\"https:\/\/journals.lww.com\/ijmr\/pages\/default.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">Krishnaswamy, 2018<\/a>).<\/li>\n<\/ul>\n\n\n\n<p>Selon les auteurs, les <strong>m\u00e9canismes biologiques<\/strong> \u00e0 l\u2019origine de ces observations restent <strong>complexes et encore peu \u00e9lucid\u00e9s<\/strong>. Des travaux suppl\u00e9mentaires sont n\u00e9cessaires afin de <strong>mieux les identifier<\/strong>.<\/p>\n\n\n\n<p><strong>Bas\u00e9 sur <\/strong>: Niu K, et al. Prim Care Diabetes. 2023 Apr;17(2):161-167.<\/p>\n\n\n\n<div class=\"block__stylized-list\">\n    <div class=\"block__stylized-list__leading\" id=\"block__stylized-list__leading\">\n        <i class=\"fa-classic fa-regular fa-pen-to-square\" aria-hidden=\"true\"><\/i>\n        <strong>M\u00e9thodologie<\/strong>\n    <\/div>\n    <div id=\"block__stylized-list__content\" style=\"display: none;\"><ul>\n<li>38 978 participants \u00e2g\u00e9s de 18 \u00e0 79 ans ont \u00e9t\u00e9 recrut\u00e9s dans cinq zones rurales de la province de Henan (3665 personnes atteintes de diab\u00e8te de type 2 et 35 133 sujets sains).<\/li>\n<li>Un questionnaire en face \u00e0 face a \u00e9t\u00e9 r\u00e9alis\u00e9 afin de recueillir des informations sur l&rsquo;\u00e2ge, le sexe, les revenus, le niveau d&rsquo;\u00e9ducation, l&rsquo;activit\u00e9 physique, le tabagisme, la consommation d&rsquo;alcool, les ant\u00e9c\u00e9dents familiaux de diab\u00e8te et les ant\u00e9c\u00e9dents personnels de maladie. Men\u00e9e sur pr\u00e8s de <strong>40 000 adultes chinois<\/strong> issus de la province de Henan, cette \u00e9tude est l\u2019<strong>une des premi\u00e8res<\/strong> \u00e0 <strong>inclure <\/strong>des participants issus de <strong>zones rurales<\/strong>.
