Aux c\u00f4t\u00e9s des facteurs g\u00e9n\u00e9tiques, hormonaux, et bien s\u00fbr de l\u2019\u00e2ge, de tr\u00e8s nombreux facteurs li\u00e9s au mode de vie interviennent, au premier rang desquels l\u2019activit\u00e9 physique et la nutrition (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/19840876\/\" target=\"_blank\" rel=\"noopener\">Rizzoli, 2010<\/a>). Une r\u00e9cente revue de la litt\u00e9rature men\u00e9e par une des meilleures \u00e9quipes dans le domaine de l\u2019os et de la nutrition (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/34242583\/\" target=\"_blank\" rel=\"noopener\">Rizoli, 2021<\/a>) dresse un inventaire exhaustif de la question. Elle pointe l\u2019importance d\u2019apports ad\u00e9quats en prot\u00e9ines, calcium et vitamine D, mais \u00e9galement en fruits et l\u00e9gumes.<\/strong><\/p>\n<p>Les auteurs ont examin\u00e9 le r\u00f4le des nutriments, des aliments et de certains r\u00e9gimes alimentaires (au sens large), avec une approche nutritionnelle et scientifique rigoureuse.<\/p>\n<h2>Produits laitiers : un effet positif sur la densit\u00e9 min\u00e9rale osseuse et le risque de fractures<\/h2>\n<p>Les produits laitiers remportent le palmar\u00e8s en termes de preuves. Leur absence de consommation est associ\u00e9e \u00e0 un risque accru d\u2019ost\u00e9oporose et de fractures quel que soit l\u2019\u00e2ge (<a href=\"http:\/\/- Opotowsky AR, Bilezikian JP. Racial differences in the effect of early milk consumption on peak and postmenopausal bone mineral density. J Bone Miner Res 2003; 18: 1978\u201388.\" target=\"_blank\" rel=\"noopener\">Opotowsky, 2003<\/a> ; <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/14760576\/\" target=\"_blank\" rel=\"noopener\">Gouldin, 2004<\/a> ; <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/17516020\/\" target=\"_blank\" rel=\"noopener\">Konstantinowicz, 2007<\/a>). Les \u00e9tudes d\u2019observation montrent qu\u2019une consommation \u00e9lev\u00e9e est associ\u00e9e \u00e0 une augmentation de la densit\u00e9 min\u00e9rale osseuse, de la solidit\u00e9 osseuse et \u00e0 un moindre risque de fracture (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/14988471\/\" target=\"_blank\" rel=\"noopener\">Matkovic, 2004<\/a>). Le b\u00e9n\u00e9fice le plus net est observ\u00e9 pour les produits laitiers ferment\u00e9s (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31603185\/\" target=\"_blank\" rel=\"noopener\">Ong, 2020<\/a>). Cet effet n\u2019est pas li\u00e9 au calcium seul mais bien \u00e0 un effet matrice de l\u2019aliment puisque, pour un apport calcique \u00e9quivalent, une suppl\u00e9mentation en calcium est moins efficace sur la densit\u00e9 min\u00e9rale osseuse qu\u2019une consommation de fromage (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/16280447\/\" target=\"_blank\" rel=\"noopener\">Cheng, 2005<\/a>). En effet, des apports \u00e9lev\u00e9s en calcium augmentent la densit\u00e9 min\u00e9rale osseuse surtout chez l\u2019enfant et l\u2019adolescent, mais il n\u2019a pas \u00e9t\u00e9 d\u00e9montr\u00e9 de r\u00e9duction du risque de fracture (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/16980314\/\" target=\"_blank\" rel=\"noopener\">Winzenberg, 2006<\/a>).<br \/>\nLa vitamine D joue \u00e9galement un r\u00f4le essentiel dans le m\u00e9tabolisme osseux. La suppl\u00e9mentation en vitamine D et en calcium r\u00e9duit le risque de fractures chez les sujets \u00e0 risque et chez les sujets \u00e2g\u00e9s (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31860103\/\" target=\"_blank\" rel=\"noopener\">Yao, 2019<\/a>).<br \/>\nLe r\u00f4le des prot\u00e9ines est \u00e9tabli par des \u00e9tudes \u00e9pid\u00e9miologiques prospectives qui montrent que des apports \u00e9lev\u00e9s sont associ\u00e9s \u00e0 une r\u00e9duction du risque fracturaire surtout pour les prot\u00e9ines laiti\u00e8res. Les interactions calcium-prot\u00e9ines et calcium-vitamine D-prot\u00e9ines semblent favorables.<\/p>\n<h2><b>Fruits et l\u00e9gumes\u00a0: une consommation r\u00e9guli\u00e8re r\u00e9duirait de 8% le risque de fractures<\/b><\/h2>\n<p>La consommation de fruits et l\u00e9gumes est \u00e9galement b\u00e9n\u00e9fique sur la densit\u00e9 min\u00e9rale osseuse. Les \u00e9tudes d\u2019observation montrent une r\u00e9duction de 8 % du risque de fracture en cas de consommation r\u00e9guli\u00e8re de fruits et de l\u00e9gumes (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31150426\/\" target=\"_blank\" rel=\"noopener\">Brondani, 2019<\/a>). Plusieurs m\u00e9canismes d\u2019action sont \u00e9voqu\u00e9s pour expliquer cet effet protecteur\u00a0:<\/p>\n<ul>\n<li>une action alcalinisante des fruits et l\u00e9gumes agissant sur l\u2019\u00e9quilibre acide-base de l\u2019organisme,<\/li>\n<li>un effet antioxydant, li\u00e9 \u00e0 la pr\u00e9sence de vitamine C, de b\u00eata-carot\u00e8ne et de vitamine K dans les fruits et l\u00e9gumes<\/li>\n<li>un effet pr\u00e9biotique sur le microbiote, susceptible d\u2019augmenter l\u2019absorption calcique.<\/li>\n<\/ul>\n<p>En effet, les \u00e9tudes soulignent que le b\u00e9n\u00e9fice de la vitamine A d\u00e9pendrait de la source car seule la prise en compte du b\u00eata-carot\u00e8ne permet d\u2019observer une relation inverse avec les fractures (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33129293\/\" target=\"_blank\" rel=\"noopener\">Charkos, 2020<\/a>). Agissant sur la synth\u00e8se de collag\u00e8ne par les ost\u00e9oblastes et inhibant l\u2019activit\u00e9 ost\u00e9oclastique via son effet antioxydant, la vitamine C est associ\u00e9e dans les \u00e9tudes d\u2019intervention \u00e0 une forte r\u00e9duction du risque de fracture du col du f\u00e9mur (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32117042\/\" target=\"_blank\" rel=\"noopener\">Zeng, 2020<\/a>). La vitamine K est aussi un candidat mais l\u2019effet, s\u2019il existe, n\u2019est que modeste.<\/p>\n<h2><strong>Le r\u00e9gime m\u00e9diterran\u00e9en, favorable pour la sant\u00e9 osseuse<\/strong><\/h2>\n<p>Enfin, l\u2019effet de plusieurs r\u00e9gimes alimentaires sur le risque d\u2019ost\u00e9oporose ont \u00e9t\u00e9 \u00e9tudi\u00e9s. Les r\u00e9gimes v\u00e9g\u00e9tarien et surtout v\u00e9g\u00e9talien, ont un effet extr\u00eamement d\u00e9l\u00e9t\u00e8re, att\u00e9nu\u00e9 par des apports \u00e9lev\u00e9s en calcium (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/19571226\/\" target=\"_blank\" rel=\"noopener\">Ho-Pham, 2009<\/a>). Les r\u00e9gimes amaigrissants ont \u00e9galement un effet d\u00e9l\u00e9t\u00e8re att\u00e9nu\u00e9 par une suppl\u00e9mentation en prot\u00e9ines, calcium, vitamine D et par l\u2019activit\u00e9 physique (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26012544\/\" target=\"_blank\" rel=\"noopener\">Zibellini, 2015\u00a0<\/a>; <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33403429\/\" target=\"_blank\" rel=\"noopener\">Paccou, 2021<\/a>). Le r\u00e9gime m\u00e9diterran\u00e9en est celui qui est le plus favorable sur l\u2019os.<\/p>\n<p><strong>Au-del\u00e0 du r\u00f4le b\u00e9n\u00e9fique largement \u00e9tabli du calcium et de la vitamine D, ce travail souligne \u00e0 nouveau l\u2019importance d\u2019une alimentation saine, vari\u00e9e, riche en fruits et l\u00e9gumes pour la pr\u00e9vention de l\u2019ost\u00e9oporose.<\/strong><\/p>\n<div class=\"block-bordered-full border-primary\">\n<p><span style=\"font-size: 14pt;\"><strong class=\"strong-text\">MESSAGES CL\u00c9S <\/strong><\/span><\/p>\n<p>&nbsp;<\/p>\n<p>En pr\u00e9vention de l\u2019ost\u00e9oporose, les auteurs recommandent une alimentation apportant :<\/p>\n<ul>\n<li>0,8 g de prot\u00e9ines\/kg\/jour (jusqu\u2019\u00e0 1,3 chez les sujets \u00e2g\u00e9s) ;<\/li>\n<li>800 mg \u00e0 1 g de calcium par jour ;<br \/>\n800 \u00e0 1 000 ui de vitamine D,<\/li>\n<li>5 portions de fruits et de l\u00e9gumes, peu de sel (\u2264 2,5g\/j).
