Nous avons, en effet, r\u00e9cemment rapport\u00e9 que cette di\u00e8te prudente pourrait r\u00e9duire de moiti\u00e9 le risque de Broncho-Pneumopathie Chronique Obstructive (BPCO).<\/p>\n<h2>Deux types de comportements alimentaires identifi\u00e9s<\/h2>\n<p>Les donn\u00e9es ont \u00e9t\u00e9 recueillies dans une large cohorte prospective de professionnels de sant\u00e9 am\u00e9ricains males, \u00e2g\u00e9s de 40 \u00e0 75 ans (Health Professionals Follow-up Study-HPFS). Ils ont r\u00e9pondu \u00e0 un questionnaire d\u00e9taill\u00e9 englobant l\u2019alimentation, le mode de vie (tabagisme, activit\u00e9 physique, poids) et les ant\u00e9c\u00e9dents m\u00e9dicaux. Des questionnaires de suivi ont \u00e9t\u00e9 envoy\u00e9s tous les deux ans par la suite. La consommation alimentaire a \u00e9t\u00e9 estim\u00e9e en 1986 puis tous les 4 ans. Les comportements alimentaires ont \u00e9t\u00e9 identifi\u00e9s \u00e0 partir des questionnaires de 1986, 1990 et 1994, en analysant la fr\u00e9quence de consommation des composants principaux. Deux types de comportements alimentaires ont ainsi \u00e9t\u00e9 identifi\u00e9s: le r\u00e9gime prudent (caract\u00e9ris\u00e9 par une forte consommation de fruits, l\u00e9gumes, poisson et c\u00e9r\u00e9ales \u00e0 grains entiers) et le r\u00e9gime occidental (forte consommation de c\u00e9r\u00e9ales raffin\u00e9es, charcuterie, viande rouge, confiserie\/desserts et frites).<\/p>\n<h2>Une r\u00e9duction de 50 % du risque de BPCO<\/h2>\n<p>Entre 1986 et 1998, parmi les 42 917 hommes, 111 ont rapport\u00e9 une BPCO nouvellement diagnostiqu\u00e9e. Le r\u00e9gime prudent et le risque de BPCO nouvellement diagnostiqu\u00e9e \u00e9taient inversement associ\u00e9s, m\u00eame apr\u00e8s ajustement pour l\u2019\u00e2ge, le tabagisme et les autres facteurs de risque de BPCO. Nous avons pu noter que plus un sujet respectait ce r\u00e9gime alimentaire, plus faible \u00e9tait son risque de BPCO \u2013 une r\u00e9duction de 50 % pour les consommateurs en meilleure sant\u00e9.<\/p>\n<p>D\u2019un autre cot\u00e9, nous avons not\u00e9 que le r\u00e9gime \u201coccidental\u201d (riche en c\u00e9r\u00e9ales raffin\u00e9es, charcuterie, viande rouge, frites et confiseries) augmentait de fa\u00e7on significative le risque de BPCO. Parmi les plus forts consommateurs d\u2019un tel r\u00e9gime, un sur cinq avaient 4 fois plus de risque de BPCO compar\u00e9s \u00e0 ceux qui s\u2019alimentaient autrement.<\/p>\n<h2>Adopter une vision plus globale de l\u2019alimentation<\/h2>\n<p>Actuellement, la BPCO est la quatri\u00e8me cause de mortalit\u00e9 en Europe et aux Etats Unis. Et comme le tabagisme augmente dans les pays \u00e9mergents, la BPCO sera la troisi\u00e8me cause de mortalit\u00e9 dans le monde entier en 2020. Si le tabagisme est le plus important facteur de risque de BPCO dans les pays industrialis\u00e9s, tous les fumeurs ne d\u00e9veloppent pas une BPCO, ce qui sugg\u00e8re l\u2019existence d\u2019autres facteurs de risque. Parmi les facteurs d\u2019environnement, des modifications alimentaires ont \u00e9t\u00e9 \u00e9voqu\u00e9es. Divers aliments ont ainsi \u00e9t\u00e9 associ\u00e9s \u00e0 la BPCO ou \u00e0 la fonction pulmonaire, cependant toutes ces \u00e9tudes ne portaient que sur des nutriments ou des aliments particuliers. Parce que les aliments sont consomm\u00e9s ensemble et qu\u2019il existe des interactions entre les diff\u00e9rents nutriments, nous avons entrepris d\u2019\u00e9tudier plut\u00f4t les comportements alimentaires, ce qui donne une vision plus globale de l\u2019alimentation.<\/p>\n<h2>Un impact b\u00e9n\u00e9fique des antioxydants<\/h2>\n<p>L\u2019association entre le r\u00e9gime prudent (englobant des fruits et l\u00e9gumes) et la r\u00e9duction du risque de BPCO nouvellement diagnostiqu\u00e9e est conforme aux \u00e9tudes \u00e9pid\u00e9miologiques ant\u00e9rieures. Dans la litt\u00e9rature, plusieurs articles sugg\u00e8rent un impact b\u00e9n\u00e9fique des antioxydants (surtout la vitamine C et, \u00e0 un moindre degr\u00e9, la vitamine E) sur la BPCO ou la fonction pulmonaire. Au contraire, un r\u00e9gime de type occidental est tr\u00e8s riche en salaisons \u00e0 forte teneur en nitrites et en aliments \u00e0 fort index glyc\u00e9mique (c\u00e9r\u00e9ales raffin\u00e9es, desserts, friandises). On a avanc\u00e9 l\u2019id\u00e9e d\u2019un lien entre hyperglyc\u00e9mie et g\u00eane respiratoire ou r\u00e9duction de la fonction pulmonaire. Beaucoup d\u2019aliments pr\u00e9sents dans le r\u00e9gime occidental (salaisons, c\u00e9r\u00e9ales raffin\u00e9es) pr\u00e9sentent une corr\u00e9lation avec la BPCO. Le r\u00e9gime de type occidental repr\u00e9sente donc un bon mod\u00e8le pour r\u00e9sumer l\u2019ensemble des effets de ces diff\u00e9rents aliments.<\/p>\n<h2>Le poids des autres facteurs de risque<\/h2>\n<p>Les comportements alimentaires vont de pair avec d\u2019autres comportements \u00e0 risque. Ainsi, nous avons rapport\u00e9 que les plus forts consommateurs d\u2019un r\u00e9gime \u201coccidental\u201d avaient \u00e9galement un indice de masse corporelle (IMC) plus \u00e9lev\u00e9, pratiquaient moins d\u2019activit\u00e9s physiques et fumaient plus que ceux consommant un r\u00e9gime \u201cprudent\u201d.<\/p>\n<p>Il faut confirmer ces r\u00e9sultats dans d\u2019autres populations, particuli\u00e8rement celles ayant des habitudes alimentaires diff\u00e9rentes. Ainsi seront fournies des preuves suppl\u00e9mentaires des b\u00e9n\u00e9fices probables d\u2019une alimentation riche en fruits et l\u00e9gumes et des risques potentiels d\u2019un r\u00e9gime de type occidental.
