A healthy diet for better cardiovascular health
Au cours des dernières années, les preuves scientifiques démontrant le lien entre alimentation et maladies cardiovasculaires se sont largement confirmées.
L’European Heart Network EHN (Réseau européen du cœur) a présenté les conclusions scientifiques dans son article Transforming European food and drink policies for cardiovascular health (2017), dans lequel il propose des objectifs en termes d’aliments et de nutriments pour la population. Ces objectifs incluent des recommandations concernant les F&L s’appuyant sur des preuves solides que consommer davantage de F&L protège contre les morts prématurées et, plus spécifiquement, contre les décès cardiovasculaires et le risque d’accident vasculaire cérébral.
L’EHN a proposé des objectifs intermédiaires et à long terme pour la population, relatifs à la consommation de F&L (tableau 1) :
[TABLEAU]Une alimentation favorisant une meilleure santé cardiovasculaire suppose de passer d’un régime à base dominante d’aliments d’origine animale à une alimentation reposant davantage sur des aliments d’origine végétale, incorporant des légumes, des fruits et des baies en grande quantité. Les produits céréaliers complets, les fruits à coque et les graines, le poisson, les légumineuses et les produits laitiers à faible teneur en matière grasse sont également importants à consommer, tout comme les huiles végétales en petite quantité.
Un tel modèle, adopté quotidiennement, limite de facto la consommation de viande rouge, de produits transformés à base de viande et d’aliments
ou de boissons pauvres en vitamines, minéraux et fibres alimentaires et/ou riches en sucres simples, acides gras saturés/trans ou sel.
Trois grands modèles alimentaires favorables à la santé cardiovasculaire ont fait l’objet de nombreuses recherches scientifiques :
- Régime méditerranéen : caractérisé par un apport important de F&L, légumineuses, produits céréaliers complets, acides gras mono insaturés
(huile d’olive) et poisson. Les études ont montré que ce régime peut diminuer la mortalité globale et cardiovasculaire ainsi que le risque
cardiovasculaire (1) - DASH (Dietary Approach to Stop Hypertension): régime à forte base d’aliments d’origine végétale, riche en fruits, légumes, céréales complètes et fruits à coque, comprenant également des produits laitiers pauvres en matières grasses, des viandes maigres, du poisson et de la volaille. Ce modèle alimentaire est associé à une amélioration de la tension artérielle et du profil lipidique et, par conséquent, à une diminution du risque de MCV (2).
- Régime alimentaire nordique sain : comprend des aliments naturellement riches en fibres, tels que les légumes, les légumineuses, les fruits (particulièrement les fruits rouges), les fruits à coque, les graines et les céréales complètes, ainsi que l’huile de colza et les produits laitiers sans ou allégés en matières grasses. Diverses études ont montré que ce régime a des effets cardiovasculaires bénéfiques (3)
Composants alimentaires augmentant le risque de MCV
Pour réduire le risque de MCV, certaines recommandations de l’EHN concernant le sucre, les matières grasses et le sel sont présentées Tableau 2.
[TABLEAU]Dans leur ensemble, les objectifs proposés ci-dessus pour la population devraient se traduire par un régime alimentaire favorisant une meilleure santé cardiovasculaire, et de faible densité énergétique, ce qui est important pour le maintien du poids et la prévention du surpoids et de l’obésité. Pour les adultes, l’EHN propose de parvenir à un indice de masse corporelle (IMC) moyen inférieur à 23 comme objectif intermédiaire et à 21 comme objectif à long terme. Un tel régime alimentaire diversifié et équilibré couvre les besoins en nutriments et des compléments alimentaires sont rarement nécessaires.
